Leptospirosis disease on the increase in Dominica

There is a small epidemic of leptospirosis in Dominica, the Environmental Health Department has revealed.

The prevalence of the disease is believed to have increased in Dominica by rats and authorities are describing the situation as critical.

There were 14 cases of the disease in 2010, a significant increase from other years, Chief Environmental Health Officer Anthony
Scotland said.

“We are averaging almost three rats per person according to a survey that has been conducted recently. We suspect that these rats are contributing to the spread of leptospirosis in the country,” Anthony noted.

He said before 2010 Dominica recorded very few cases of the disease.

“We were just used to getting one case but we have seen actually a small epidemic of the disease in Dominica,” Scotland said.

The Environmental Health Department said it’s not taking the issue lightly. A campaign to manage rodents in Dominica is now in its second phase. Environmental health officials are starting off the programme as a pilot project in the Roseau health district.

“Part of the program is setting the rat baits, putting down your bait stations and applying rodenticide to them. Also we have ongoing clean-up campaigns of all the blocks,” Scotland said.

    I think it is a major epedemic,when you walk to the streets of roseau it is ashame of the kind of stench you smell.The bus drivers are part of it.Look @ the condition of some of the restaurants that are in the city.The small garbage piles that we see daily.the rats must take over.

    Every Dominican should be concerned about the increase in cases of persons infected with Leptospirosis in Dominica. This is not very good news. According to the DNO Report the Chief Environment Health Officer refers to the outbreak as a small epidemic. The report states that there is approximately 3 Rats per 1 person in Dominica. The report also states that the Environmental Health Unit(EHU) is taking necessary steps to curb the spread by setting rat traps and bait. My questions to the EHU are (1) where is the spread of this disease concentrated? (2) are patients coming from different parts of Dominica? (3) are there plans to undertake a education campaign to support the campaign of setting those rat traps and bait?

    I do hope that as part of the campaign Dominicans are educated about the importance of our snakes in the food chain. Snakes are a natural predator of rats yet we go on killing them indiscrimminately. Stop the indiscrimminate killing of snakes. Unlike rats, they are not known to carry the deadly leptospirosis disease that the rat carry…

    I am a big rat not because i roam in gutters human beings have to degrade me. That the way I live, I look for scraps of food in the gutter and in your gabbage. If you properly dispose your gabbage there will be no food for me. I could bite your toe when your are sleeping or your lee, so don’t blame me. Your all are too bi- ass. What about the dog and cats which seat on your settee and your are still preparing food to serve for the public. what about when they peepee and poopoo and don’t use tissue and you touch the chair and touch a sandwich and hot dog.. I honourable RAT is not nasty that my way of life. Human being is too dam nasty.     What did we expect. There is so much rubbish scattered all over the place.

    There are no proper designated places for residents to put their rubbish for collection, so they leave it anywhere. There are no bins, so people drop their rubbish anywhere. There are not enough public conveniences, so men use any waste ground to urinate.

    I leave presently in Europe. In one of the most cleannest country in the World. not boasting just mentioning.

    I remember in the early 70s in Dominica, when there were many people employed to maintain the streets and take care of the gabbage and ensure that everywhere were kept clean

    Still remember a particular lady from fond cole, she always came every ealy morning to make sure that the road was swept and kept clean. (sometimes she was not in a very good mood, but she did the job) :lol:

    How in gods name do you think I feel to return to DA, I have a young 4 months old son and i am just scared, that something may happen to us if we set foot in DA. maybe I am exaggerating but i am commenting, based on what i am reading

      Yeah I can remember while growing up there was a man in Salisbury called ‘So-so-wi’ who used to clean the gutters and garbage from the village…he has passed…i am not sure if anyone has taken up his duties.

      I totally agree with Anonymous, I too lived abroad for over 15 yrs and whenever I spoke of my country I did so proudly…even when reading the comments of persons online or listening to our radio programs I never once had an ill thought of returning to my country.

      So Zuma I find you well ‘fresh’ …need you be so naive and stupid to allow yourself to think that harm would come to you and your 4 month old if you came home. Well let me assure you not a damn thing would happen if you did…our country is NOT plagued by pollution…lucky for you where you live may not have issues with garbage collection…but there are alot more issues on rubbish and pollution in Europe than D.A …trust me and our babies are beautiful and healthy and suffer from a lot less rashes and illnesses than the babies born in your cleanest country of the world.

    Do we still have Environmental Health Officers in Dominica? I remember growing up, they would make visits to our shop unannounced. They would inspect our building, even I was a child then I can still remember some of the things she would look for. We sold bread, she made sure we had the proper stuff to hold the bread, ( it had to be a tong – no plastic bag business). The surroundings had to be clean . Properly bathroom/toilet facility, our garbage disposal had to be certain distant from our shop, and it had to be burnt regularly to keep away flies, no containers should be around the shop collecting water to breed mosquito etc etc.

    I hope that still exist, cause if rats is too common in DA then thats just bad business for the country, especially roseau. Imagine a tourist ship in port and there are more rats than tourists

    City council I am tired of playing hopscotch to avoid stepping in DOG AND HUMAN feces in Roseau, especially around the CITY COUNCIL AREA. They need to start impounding dogs and charging dog owners. Some of the owners in Roseau simply wash the feces from the yard straight unto the sidewalk or drains…….totally disgusting.

    This may well result from the growth of fast food outlets in Roseau and Portsmouth. Food and drink containers are discarded anywhere.

    Once solution may be to place more bins around the key spots, and to employ more street cleaners.

    This is an issue that I have taken up with many people already both in the health sector at the DSWC the Roseau City council and the likes. I knew it was coming sooner or rather than later. Dominica is the Nature Island we say but it is quickly becoming the Nasty Island – and since when we came like that – we use to see people with cocoye brooms sweeping their surroundings – picking up garbage and the likes -now we to big and sophisticated for that – we speak of the next level the next level my farorite part of my anotomy -and how we ahead of other islands -crazy-say that to people who do not travel my friend;

    Each and everyone of us have an individual responsibility for the chaos happening at all levels of our society -the lawlessness, the nastiness, the disrepect, the flithyness, the laziness -Dominica is the island with the most …..nesses; it is really pathetic and Honorable Pierre Charles of Blesseth memory is right -we wear our ignorance like a badge of honor and with arrogance at that- we are becoming a shameless bunch of nasty and arrogant people- and we wonder why Carnival pulled out – take a good look at Roseau my friend -straw dogs; homeless people ; garbage; nasty drains; resturants and bars without washroom facilities ; rum shops on every street corner with loud musicetc etc. what a place and we say we are making sound and steady progress!!!!!

    Reminds me of NY, when I went to Brooklyn I thought I wouldnt be surprised if I saw a rat crossing the street with a chicken leg. so many rats ( huge rats). i realized that rats were mostly common around all the fast food resturants who do not dispose off their garbage properly. Could that be a problem in DA? ( maybe Roseau)

    I am surprised to see how the majority of Dominicans have become so nasty with their environmental surroundings. Roseau, in particular is the biggest surpise to me. The drains stinks and people litter and urinate openly anywhere and anyhow. There are those who openly dig their noses as though they are digging for gold!! The other day, I saw a hug rat in the vicinity to a so called restaurant. I personally would rather starve than eat out!!

    Most Dominicans abuse the freedom they have and think they can do anything. Clealiness never ends but nastiness comes to an end and when it cant take any more, it’s pay back time which could be detrimental to a country and its people. Governments should stop turning a blind eye to these problems for the sake of votes!!

    while we can’t totally blame (i said totally) the office of the environmental health we have to hold them responsible for the state in which we are now….some of the areas in question need to be examined very closely,for example the building between Green’s wholesale and Cocorico on Kennedy ave is truely an example of a breeding ground for rats, rodents and all sorts of the most disguting creatures one can possibly think of.Ppl from all over dump all sorts of garbage there, men stop off to enter this dump to use as a public convienence and this is becoming really unbearable cause when the wind blows the scent coming from the said building is like a knock out punch from mike tyson…. please the the love of our country and our health (those of us within the area) can the health officers do their jobs and find who is responsible for that disgusting dump which stands there polluting the city.

    Don’t blame Solid waste coorporation.We ain’t see nothing yet!!. Our life styyle is changing rapidly and so is our environment. Dominica is rapidly becoming a BIG GARBAGE DUMP with the proliferation of these cheap imported chinese products which end up all around the island as trash. Trash harbors RODENTS.

    Also these FAST FOOD companies and the greassy packages also help in worsening the problem.
    The more we move away from a healthy and natural lifestyle is the worse it’s going to get.
    I’ve lived in many BIG CITIES in the U.S and these BIG CITY RATS running out of dumpsters which are located behind the fast food retaurants are big as manicou. Also CITYrats don’t have natural predetors like cats and snakes to control them…This is serious we need to reduce trash output.

    CLEANLINESS IS NEXT TO GODLINESS. COMMUNITIES, society on a whole have begun so SPIRITUALLY DEAD that love for the ENVIRONMENT, cleanliness of our environment means nothing to us any more. People have not come to the realization that human beings are part and parcel of the environment and if the place is not clean, it is the human beings who will succumb to all kinds of diseases due to irresponsible behaviour like dumping rubbish anywhere and everywhere.

    Why would someone dispose of his or her rubbish anywhere and expect someone else to clean it for them? Part of the problem stems from the ridiculing, ostracising discarding of those persons who consistently advocate and speak about preserving, conserving, cleaning the environment. Those in authority has made it their business to bash the “ENVIRONMENTALISTS’, claiming that those ‘FELLAS always talking nonsense. So from the ‘TOP’ there is little regard, NO LOVE for our pristine, unique, green beautiful environment.

    Remember when the idea for ADOPTING and implementing A CLEAN AND GREEN STRATEGY FOR DOMINICA’S development? Foolishly, a Minister in Govt., and he got maximum support from his colleagues, laughed at the idea, and indicated that because the surname of the leader of the Opposition was ‘GREEN’ that’s why he brought the idea forward. What an ignoramus statement. To even think of such stupidity, and to boldly and publicly tell that to the public, says plenty of how our people in authority think about cleanliness and taking care of our ENVIRONMENT.

    Is so Dominica come? Self-esteem, personal cleanliness have gone down the drain. Dominica is Spiritually and culturally dead. Just check again the issue of the ‘ASBESTOS’ at the back of SMA that could have serious health effects for the youth and other persons. Just check. the cancerous fumes affecting the health of an entire village and its citizens of Layou. Who supported that dangerous action?     Leptospirosis is an infectious disease caused by a type of bacteria called a spirochete. Leptospirosis can be transmitted by many animals such as rats, skunks, opossums, raccoons, foxes, and other vermin. It is transmitted though contact with infected soil or water. The soil or water is contaminated with the waste products of an infected animal. People contract the disease by either ingesting contaminated food or water or by broken skin and mucous membrane (eyes, nose, sinuses, mouth) contact with the contaminated water or soil.

    Leptospirosis symptoms and signs….

    The illness typically progresses through two phases:

    The first phase of nonspecific flu-like symptoms includes headaches, muscle aches, eye pain with bright lights, followed by chills and fever. Watering and redness of the eyes occurs and symptoms seem to improve by the fifth to ninth day.

    The second phase begins after a few days of feeling well.

    The initial symptoms recur with fever and aching with stiffness of the neck. Some patients develop serious inflammation of the nerves to the eyes, brain, spinal column (meningitis), or other nerves. Right upper area abdominal pain may occur. Less common symptoms relate to disease of the liver, lungs, kidneys, and heart.

    Leptospirosis associated with liver and kidney disease is called Weil’s syndrome and is characterized by yellowing of the eyes (jaundice). Patients with Weil’s syndrome can also develop kidney disease and have more serious involvement of the organs affected.

    A few things.

    First of all, the article could have given a little more detail explaining what is leptospirosis, its symptoms etc.

    Secondly, “The prevalence of the disease is believed to have increased in Dominica by rats and authorities are describing the situation as critical”. Well if it is rat bite poisoning how else can it be spread? Stupes

    Finally, rat infestation is a neighborhood problem and as such has to be fought by everyone not just some. Proper garbage disposal is critical – if there is no food for the rats to eat then they will starve.

      Lepto is spread more by rat urine and faeces than by bites. If contaminated urine gets into the water system???? or small pools and ponds then the disease can be caught by humans as it is more waterborne than any other means of transmission.
      Dominicans are too nasty and unhygienic in their food prep.
      Now you have to pay the price.
      Dog faeces is another issue which is just waiting to take a hold. Then there is the cough/spitum disposal which is all too common in Dominica. Do not coughin people face and spit in the street.

        your comment on cough and spit really hit a nerve cus i agree with you on that one for sure. there are times in roseau when u walking around and lo and behold you seeing a bit disgusting phlegm coughed up from somebody throat and sitting on the pavement. Disgusting to say the least! additionally, some people have a bad habit of just pulling up in any corner of roseau and just pee-pee-ing like is their toilet-that is also unsanitary! I once had a friend who had a bar in lagon, roseau who invited in the little kitchen where she normally fries bakes and chicken etc to sell. Boy, let me tell you, after i come out there, i decided never to buy bakes or nothing except bottled drinks there again! The size of those rats was mindboggling and the best part is they all seem so tame! I tell you the rats were in no hurry to run in any hole to hide, they were just scurrying around like they were more than accustome to the lady and her bakes. people should be careful where they buy food believe me!

      Well said hmm, the article is lacking alot of important information and I mean thank God for DNO because we might have never even gotten that briefing. PLEASE READ BELOW IF YOU NEED MORE INFO:

      I honestly haven’t seen a rat for a long time. But if anyone got them they are nasty and you need to ensure that you keep your homes squeecky clean…they will pee and shit on everything so make sure to wash your cups, plates cuttlery everything from your cupboards before use and whatever you do never leave dirty dishes in your sink at nights…even though you dropping tired wash up and disinfect before you go to bed…it’s better than ending up with leptospirosis: If you have babies you done know you have to be on top of the cleaning…get the clorox wipes they cost a pretty penny but trust me that will save you more than having to go to PMH…that place scares the daylight out of me…sorry…Oh last thing…your pets especiallly dogs can contract the disease ok people. So you know how we put a bowl of left overs out for them and it stays there over night…rats pass and eat, shit, pee in there bowl and there you go need I say more…I’m not a fan of pets but if you have them care for them properly…wash the bowl before each feed or after..just make sure its clean.

      TREATMENT:
      The treatment of leptospirosis involves high doses of antibiotics. Antibiotic treatment (doxycycline [Vibramycin, Oracea, Adoxa, Atridox], penicillin) is most effective when initiated early in the course of the illness. Severely ill patients may need hospitalization for IV fluid and antibiotic treatment. Severe liver and kidney manifestations of the infection may require intensive medical care and sometimes dialysis treatment. However, even in severe cases, liver and kidney function often does return after recovery from the illness.
      A vaccine for leptospirosis is available and used in some countries in Europe and Asia. It must be given every year like a flu shot. A longer-acting vaccine is being investigated in Cuba. It is not currently available in the United States.

      PETS CAN ALSO CONTRACT LEPTOSPIROSIS:
      Your pet can contract it in the same ways you can (ingesting contaminated soil, water, or through skin wounds). Your pet may exhibit vomiting, refusal to eat, weight loss, decreased activity, muscle pains, or stiffness.
      If your pet is diagnosed with leptospirosis, you must be careful to try to prevent exposure to yourself or other household members. Remember to wash your hands frequently with soap and water after cleaning up waste from your pet. If possible, use latex or rubber gloves to do the job of cleanup. Use a diluted (1:10 parts) bleach solution to clean surfaces where pet wastes may have contaminated. And make sure your pet receives the full course of antibiotic treatment that is prescribed by your vet.

This is serious, just wish that Mr. Scotland could have gone a bit more in-depth on this, Roseau and other areas need serious methodology in cleaning and remaining clean. The biggest culprit of the whole thing revolve around ourselves, the way we dispose of our waste, garbage etc.. are factors which encourage rats!

      Unfurtunately the increase of this disease in Dominica may not neccessarily have even come from rats in Dominica itself but by rats outside of Dominica. I guess what I am trying to say is that infected items now travel around the world on a daily basis and therefore this disease can start in one area and be transported to another totally different part of the world within a few hours. For example, all canned drinks sit in a wharehouse or two before delivery, purchase and consumption by consumers. Wharehouses are favourite hangout areas for rats and mice who urinate on top of these crates of drinks which might be shipped and distributed to retailers within hours. Sadly, there has been actual recorded cases of leptospirosis which had been contracted through the use of canned drinks. It is very important to wash or wipe off the top of these drinks as best as possible before putting them to your mouth. But having said that, it is also important to thoroughly clean off all products where possible because almost all of our imported products sit in a wharehouse at some point before getting to us, and rats are known to love wharehouses.

    People keep your environment clean and free of rodent buffets..

    DSWC is just as culpable here with there lackadaisical approach to garbage collection. Their stop and go operations contribute immensely to the exploding rodent populations around the island.

    Thanks to the EHD for sounding the alarm bell early! It should have come a lot earlier from the ministry of health!!!!!

      I agree with you here, I sometimes wonder if these guys (which we need) even understands their task, the importance of it and how to carry it out, sometimes I see them out on their routine gloves, no gloves, mask, no mask and torn plastic bags, boxes with garbage the works and the worst is on occasions (too often) garbage can remain in certain areas for 3-4 days. We still have work to do and developing a system that works more seamlessly!

      I share the same view as you. This is very serious. We also need to care for these people that does garbage collection. We need to provide them with the necessary safety gears for this job. I hope at present, this garbage collection guys are provided with health insurance, because i see them with no gears…(just wondering..if not they should have; it’s a NEED). We as a people can’t solely depend on the garbage disposal people. We need to keep our surroundings, communities and homes as clean as possible. We can’t make our homes a home for rats. So please people, let’s practise the act of cleanliness. If everyone keeps their surroundings and communities clean…Do you know the vast impact this will make? It will keep away rodents and mosquitoes.

    Who is surprised by that development! One sees huge monster rats strolling around Roseau in these filthy drains in the bright morning. They are out in squadrons at nights. It is time that those in charge pay attention to these matters rather than to bashing Thompson or Gabu. There is work to be done fellas
