I think it\u2019s fair to say that increasingly in Dominica, we are realizing that some conditions arrive without warning, and some don\u2019t even threaten us, they just go straight ahead and kill us.\u00a0 Perhaps we can just briefly discuss several of these silent conditions.<\/p>\n

1) TYPE 2 DIABETES<\/strong><\/p>\n

This is the type of diabetes which is also called maturity \u2013 or adult \u2013onset diabetes, for reasons that the name suggests.\u00a0 Everyone knows that this is one of the most prevalent diseases in Dominica.\u00a0 Many people have it and don\u2019t know it, but the symptoms will eventually catch up with you.<\/p>\n

WHAT ARE THE SYMPTOMS<\/p>\n

\u2022\u00a0\u00a0 \u00a0Extreme thirst
\n\u2022\u00a0\u00a0 \u00a0Dry mouth
\n\u2022\u00a0\u00a0 \u00a0Increased urination
\n\u2022\u00a0\u00a0 \u00a0Blurred vision
\n\u2022\u00a0\u00a0 \u00a0Weight loss
\n\u2022\u00a0\u00a0 \u00a0Eating more than usual (polyphagia)<\/p>\n

Type 2 Diabetes is related to insulin resistance, when the body is unable to properly use insulin to control blood sugar levels. Or the pancreas may just not be producing enough insulin to deal with the level of sugar (glucose) in your blood.<\/p>\n

Any of the symptoms above should prompt you to get your blood or urine checked, especially if you have a family history.<\/p>\n

You will want to avoid stroke, amputations, heart attack and other complications that it poses.\u00a0 Keep in mind that diabetics often get silent heart attacks \u2013 without the pain, et cetera.<\/p>\n

RISK FOR DIABETES<\/p>\n

\u2022\u00a0\u00a0 \u00a0Being overweight or obese
\n\u2022\u00a0\u00a0 \u00a0Women who developed diabetes during pregnancy
\n\u2022\u00a0\u00a0 \u00a0Family history<\/p>\n

2) HEART DISEASE<\/strong><\/p>\n

Let\u2019s call this public enemy no.1 for men and women, because it is the leading cause of death in so many places, including here.<\/p>\n

SIGNS
\nOften, the first sign is not crushing chest pain, especially for women. For them, signs may include: bloating, gastrointestinal upset, back pain, arm pain, nausea, and sweating.<\/p>\n

When chest pain does occur, it is usually described as a very dull, achy, heaviness, like someone is sitting on your chest.<\/p>\n

You may not have chest pain, but instead:
\n\u2022\u00a0\u00a0 \u00a0Fatigue
\n\u2022\u00a0\u00a0 \u00a0Perspiration
\n\u2022\u00a0\u00a0 \u00a0Nausea
\n\u2022\u00a0\u00a0 \u00a0Shortness of breath<\/p>\n

PREVENTIVE MEASURES<\/p>\n

\u2022\u00a0\u00a0 \u00a0Regular exercise
\n\u2022\u00a0\u00a0 \u00a0Eating a healthy diet
\n\u2022\u00a0\u00a0 \u00a0Avoid smoking or STOP IT.
\n75 mg to 150 mg of Aspirin a day can also help.<\/p>\n

3) MELANOMA<\/strong><\/p>\n

You may think that this skin cancer is confined to white people, but if memory serves me correctly, the great Robert Nester Marley had this in his toe, and it metastasized (spread) to his brain and did him in.<\/p>\n

The thing is, people don\u2019t look at their moles at all and even those that do don\u2019t notice subtle changes.<\/p>\n

It is mostly a disease of excessive exposure to the sun in lighter skinned people, but in people of African decent, melanomas may not be in sun-exposed areas.\u00a0 We find them in the mouth, under the finger- or toe-nails or in the genital areas.\u00a0 So check yourself monthly at home to look for irregular lesions that are growing and changing.<\/p>\n

Look for these ABCD\u2019s in moles:<\/p>\n

Asymmetry of moles, where one half is different from another.
\nBorder irregularity; the edges of melanomas are usually ragged and jagged.
\nColour, because melanomas often have a variety of colours within the same mole.
\nDiameter, as melanomas continue to grow.<\/p>\n

PREVENTION<\/p>\n

\u2022\u00a0\u00a0 \u00a0Avoid sun between 10:00a.m. through 4:00p.m.
\n\u2022\u00a0\u00a0 \u00a0Use a broad-spectrum sunscreen.
\n\u2022\u00a0\u00a0 \u00a0Wear a hat and clothing with a tight weave to block UV light.<\/p>\n

4) HIGH BLOOD PRESSURE<\/strong><\/p>\n

I feel like a broken record talking about this.<\/p>\n

Most people with high blood pressure don\u2019t know it because there are usually no symptoms.\u00a0 Sure, the occasional person will get a headache, or feel dizzy, but usually there is NO SYMPTOM OR SIGN.<\/p>\n

It\u2019s important to know that high blood pressure (hypertension) can affect children, young people and adolescents too.<\/p>\n

DIAGNOSIS<\/p>\n

The only way to tell for sure that your blood pressure is high is to have it checked.\u00a0 Your blood pressure should be 119\/79 or lower, according to the American Heart Association, and that is what we go by, folks.<\/p>\n

Uncontrolled hypertension can lead to stroke, heart attack, heart failure, or kidney failure.<\/p>\n

HOW TO KEEP IT UNDER CONTROL<\/p>\n

\u2022\u00a0\u00a0 \u00a0A LOW CALORIE, LOW-SALT DIET
\n\u2022\u00a0\u00a0 \u00a0Exercise, especially aerobic, like walking.
\n\u2022\u00a0\u00a0 \u00a0Stress reduction
\n\u2022\u00a0\u00a0 \u00a0Don\u2019t SMOKE, quit smoking, and avoid second hand smoking<\/p>\n

If lifestyle changes don\u2019t work, medication is the next step.\u00a0 So make your choice folks, at a time when it can be made.<\/p>\n

5) GENITAL HERPES<\/strong><\/p>\n

This is an \u2018everywhere, everyone\u2019 disease that is very common.\u00a0 Yet early symptoms are so subtle that people often don\u2019t recognize them as a problem and then once many people have established disease, they may get no outbreaks and are asymptomatic.\u00a0 In fact, a large number of people who acquire herpes do so from someone who had no sign or symptom.<\/p>\n

If you have recurring symptoms below the belt that are unexplained, you may have herpes (Herpes Simplex II). Painful, recurrent vesicular lesions that break down and leave no scar, is the key.<\/p>\n

TREATMENT<\/p>\n

The best we can do is a cream or ointment for outbreaks, and a tablet for outbreaks and to keep it at bay.\u00a0 There is no cure. And even when you take tablets to prevent outbreaks, you can still infect a person.<\/p>\n

Condom use is yet again of vast importance.<\/p>\n

6) GLAUCOMA<\/strong><\/p>\n

Glaucoma is a painless, gradual loss of vision.\u00a0 The most common type is primary open-angle glaucoma, and about half the people it affects may be unaware that they have this potentially blinding disease, because of the lack of symptoms.<\/p>\n

It generally affects your peripheral vision, constricting it so slowly that you don\u2019t know you\u2019re missing it.<\/p>\n

WHAT HAPPENS<\/p>\n

In glaucoma, the optic nerve is damaged.\u00a0 It can be associated with elevated pressure inside the eye and can lead to visual loss.\u00a0 It is the no.1 cause of visual loss in Dominica.<\/p>\n

Early diagnosis and treatment can preserve your sight.<\/p>\n

\u2022\u00a0\u00a0 \u00a0Eye drops
\n\u2022\u00a0\u00a0 \u00a0Surgery<\/p>\n

RISK FACTORS<\/p>\n

\u2022\u00a0\u00a0 \u00a0Family history of glaucoma
\n\u2022\u00a0\u00a0 \u00a0African decent
\n\u2022\u00a0\u00a0 \u00a0Increasing age
\n\u2022\u00a0\u00a0 \u00a0Elevated eye pressure<\/p>\n

7) HIGH CHOLESTEROL<\/strong><\/p>\n

As everyone knows, this is a major risk factor for heart disease, but again without symptoms.\u00a0 Most people don\u2019t know theirs is high unless they get a blood test as part of their annual physical.<\/p>\n

TYPES OF CHOLESTEROL<\/p>\n

For the purpose of this article, let\u2019s tell you that there is Total Cholesterol,
\nGood Cholesterol (HDL) and Bad Cholesterol (LDL). LDL and total ones need to be low, the good one (HDL) needs to be high.<\/p>\n

LDL should be less than 130. The lower the better. HDL should be 40 or higher, and total should be less than 200, the number above which heart problems start to occur. You may see ranges from the local labs which are more, shall we say, liberal, but Western and international ranges tend to be tougher. I prefer this.<\/p>\n

Cholesterol forms plaque in your arteries supplying your heart, narrowing them and obstructing flow of blood.\u00a0 Without nutrients and oxygen, heart muscles dies; heart attack is the result.<\/p>\n

PREVENTING HIGH CHOLESTEROL<\/p>\n

\u2022\u00a0\u00a0 \u00a0Eat foods low in saturated fat and cholesterol
\n\u2022\u00a0\u00a0 \u00a0Lose weight if you need to
\n\u2022\u00a0\u00a0 \u00a0Exercise<\/p>\n

If these are not working, medicines can help.<\/p>\n
