{"id":67550,"date":"2012-07-04T11:01:58","date_gmt":"2012-07-04T15:01:58","guid":{"rendered":"http:\/\/dominicanewsonline.com\/news\/?p=67550"},"modified":"2012-07-04T11:01:58","modified_gmt":"2012-07-04T15:01:58","slug":"health-talk-urinary-tract-infection-uti-2","status":"publish","type":"post","link":"https:\/\/dominicanewsonline.com\/news\/homepage\/columns\/health-talk\/health-talk-urinary-tract-infection-uti-2\/","title":{"rendered":"HEALTH TALK: Urinary tract infection (UTI)"},"content":{"rendered":"<figure id=\"attachment_62679\" aria-describedby=\"caption-attachment-62679\" style=\"width: 300px\" class=\"wp-caption alignleft\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-62679\" title=\"doc\" src=\"http:\/\/dominicanewsonline.com\/news\/wp-content\/uploads\/2012\/05\/doc-300x225.jpg\" alt=\"\" width=\"300\" height=\"225\" srcset=\"https:\/\/dominicanewsonline.com\/news\/wp-content\/uploads\/2012\/05\/doc-300x225.jpg 300w, https:\/\/dominicanewsonline.com\/news\/wp-content\/uploads\/2012\/05\/doc-642x481.jpg 642w, https:\/\/dominicanewsonline.com\/news\/wp-content\/uploads\/2012\/05\/doc-150x112.jpg 150w, https:\/\/dominicanewsonline.com\/news\/wp-content\/uploads\/2012\/05\/doc.jpg 842w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><figcaption id=\"caption-attachment-62679\" class=\"wp-caption-text\">Dr. Victor Emanuel MD<\/figcaption><\/figure>\n<p><strong>OVERVIEW<\/strong><\/p>\n<p>A UTI is an infection that starts in your urinary system.\u00a0 UTI\u2019s can be painful and annoying, and become a serious health problem if the infection spreads to your kidneys.<\/p>\n<p>Women are most at risk for a UTI.\u00a0 In fact, 20% of women will likely develop one during their lifetime, and many will experience more than one.<\/p>\n<p>The urinary system is composed of the kidneys, ureter, bladder and urethra, all of which play a role in removing waste from the body.\u00a0 The kidneys, a pair of bean-shaped organs in your upper abdomen toward the back, filter waste from your blood.\u00a0 The ureters are the tubes which carry urine from the kidneys to your bladder, where it is stored until it exits the body through the urethra.\u00a0 All of these components can become infected, but most infections involve the lower tract \u2013 the urethra and the bladder.<\/p>\n<p>UTI\u2019s are typically treated with antibiotics.\u00a0 But you can take steps to reduce your chance of getting a UTI in the first place.<\/p>\n<p><strong>SIGNS AND SYMPTOMS<\/strong><\/p>\n<p>Everyone with a UTI doesn\u2019t necessarily develop recognizable signs and symptoms, but most people have some.\u00a0 They include:<\/p>\n<p>\u2022\u00a0\u00a0\u00a0 A burning sensation when urinating<br \/>\n\u2022\u00a0\u00a0\u00a0 A strong, persistent urge to urinate (to pee)<br \/>\n\u2022\u00a0\u00a0\u00a0 Passing frequent small amounts of urine. (Large amounts may be a sign of diabetes)<br \/>\n\u2022\u00a0\u00a0\u00a0 Blood in the urine (hematuria), or cloudy, strong-smelling urine.\u00a0 Pink urine may be a sign of the slight bleeding we\u2019re talking about.<\/p>\n<p>Each type of UTI may result in more specific signs and symptoms, depending on which part of your urinary tract is infected.<\/p>\n<p>Acute pyelonephritis.\u00a0 Infection can spread from your bladder to your kidneys.\u00a0 Of course, your ureters will also be involved, hence the \u201cpyelo\u201d portion of the term.\u00a0 The \u201cnephritis\u201d comes from the kidney involvement.\u00a0 Kidney infection can cause high fever, shaking chills, nausea or vomiting, and flank pain.<\/p>\n<p>Cystitis.\u00a0 Infection or inflammation of the bladder may result in pressure in the pelvis (housing your reproductive organs) and lower abdomen, and strong-smelling urine.<\/p>\n<p>Urethritis.\u00a0 Inflammation or infection of the urethra leads to burning with urination and sometimes pus in your urine.\u00a0 In men, a discharge from the penis can result from urethritis.<\/p>\n<p><strong>WHAT CAUSES A UTI?<\/strong><\/p>\n<p>Typically UTI\u2019s occur when bacteria enter the urinary tract through the urethra and begin to multiply in the bladder.<\/p>\n<p>Bacteria in the urine don\u2019t always signify an infection.\u00a0 Some people, especially older adults, may have bacteria in the bladder that cause no harm or symptoms.\u00a0 This is known as asymptomatic bacteuria.<\/p>\n<p>Cystitis may occur in women after sexual intercourse. But even girls and women who are not sexually active are susceptible to lower UTI\u2019s because the anus is so close to the female urethra.<\/p>\n<p>Most cases are caused by E.coli (Escherichia coli), a species of bacteria commonly found in the gastro-intestinal tract, (the colon).<\/p>\n<p>In urethritis, the same organisms that infect the kidney and bladder can infect the urethra.\u00a0 In addition, because of the female urethra\u2019s proximity to the vagina, sexually transmitted infections (STI\u2019s) such as herpes simplex virus and chlamydia, also are possible causes of urethritis.\u00a0 Some organisms that infect the urethra can easily spread to the vagina and cause an STI.<\/p>\n<p>In men, urethritis often is the result of bacteria acquired through sexual contact.\u00a0 The majority of these infections are caused by gonorrhea and chlamydia.<\/p>\n<p><strong>RISK FACTORS<\/strong><\/p>\n<p>Some people seem to be more likely than others to develop UTI\u2019s.\u00a0 Remember, one in five women will get at least one over a lifetime.\u00a0 One key reason is because they have a shorter urethra than men have; bacteria have a shorter distance to travel to reach the bladder.<\/p>\n<p>Sexually active women tend to have more UTI\u2019s.\u00a0 Intercourse can irritate the urethra, allowing germs to more easily travel through the urethra into the bladder.\u00a0 Women who use diaphragms for birth control may also be at high risk.\u00a0 After menopause, UTI\u2019s may become more common because vaginal, urethral and base-of-the-bladder tissues become thinner and more fragile due to loss of estrogen.<\/p>\n<p>Other risk factors include:<br \/>\n\u2022\u00a0\u00a0\u00a0 Anything that impedes or obstructs the flow of urine, such as an enlarged prostate or a kidney stone.<br \/>\n\u2022\u00a0\u00a0\u00a0 Diabetes and other illnesses that impair the immune system.<br \/>\n\u2022\u00a0\u00a0\u00a0 Medications that lower immunity<br \/>\n\u2022\u00a0\u00a0\u00a0 Prolonged use of tubes (catheters) in the bladder<\/p>\n<p><strong>DIAGNOSIS<\/strong><\/p>\n<p>If you have symptoms of a UTI, see your doctor.\u00a0 If he suspects you have a UTI, an analysis and culture of a sample of your urine will be done.\u00a0 Although no simple test can differentiate between an upper and lower UTI, fever and flank pain indicate that the infection is likely to be in your kidneys.<\/p>\n<p><strong>ANY COMPLICATIONS?<\/strong><\/p>\n<p>UTI\u2019s rarely lead to any further problem when treated promptly and properly.\u00a0 But left untreated, a UTI can become more serious than a set of uncomfortable symptoms.<\/p>\n<p>Untreated UTI\u2019s can lead to acute or chronic pyolonephritis, which could cause permanent kidney damage.\u00a0 Young children and older adults are at the greatest risk for kidney damage due to UTI\u2019s because their symptoms are often overlooked or mistaken for other conditions.\u00a0 Pregnant women with UTI\u2019s may have an increased risk of delivering low birth weight or premature babies.<\/p>\n<p><strong>TREATMENT<\/strong><\/p>\n<p>Antibiotics are the first line of treatment.\u00a0 Which one depends on the bacteria found in your urine tests, and on your health condition.<\/p>\n<p>Usually, UTI symptoms clear up within a few days of treatment.\u00a0 But sometimes antibiotics may be needed for a week or more.\u00a0 Whether given for three or seven days, always take the full course to ensure the infection is completely eradicated!<\/p>\n<p>If you have recurrent UTI\u2019s, you may receive short courses of antibiotics or a longer course of antibiotic treatment, as part of a self-treatment program.\u00a0 For a woman with infections related to sexual activity, a single dose of antibiotic post-sex may be recommended.<\/p>\n<p>For severe UTI\u2019s, hospitalization and treatment with intravenous antibiotics may be necessary.<\/p>\n<p><strong>CAN I PREVENT THIS?<\/strong><\/p>\n<p>You can take steps to reduce your risk of UTI.\u00a0 Women in particular may benefit from the following.<\/p>\n<p>\u2022\u00a0\u00a0\u00a0 Drink plenty of fluids, especially water.\u00a0 Cranberry juice seems to have infection-fighting properties.<br \/>\n\u2022\u00a0\u00a0\u00a0 Urinate frequently.\u00a0 When you feel the urge to go, go.\u00a0 Don\u2019t retain your urine for long periods.<br \/>\n\u2022\u00a0\u00a0\u00a0 Wipe from front to back after urinating or passing stool, so as not to contaminate your vagina and urethra with bacteria from the anal region.<br \/>\n\u2022\u00a0\u00a0\u00a0 Empty your bladder as soon as possible after intercourse. Drink a full glass of water to help flush bacteria.<br \/>\n\u2022\u00a0\u00a0\u00a0 Avoid potentially irritating feminine products.\u00a0 Douching and powders can irritate the urethra and really are not necessary to use.<\/p>\n<p>Be not afraid if you feel you have symptoms of a UTI.
